w:sdt inline-level structured document tag

An inline w:sdt is a run-level structured document tag whose ordered children are its properties, optional end properties, and content. The issue #582 pilot preserves an unchanged inline SDT as one opaque semantic boundary during rebuild; it does not author or edit controls and makes no claim about row or cell SDTs.
