Proofing error anchors

w:proofErr anchors mark spelling or grammar proofing state. They are consumer-rewritable metadata and carry no document content, so safe-docx treats a paragraph whose only children are proofing anchors as an empty paragraph for comparison. Rebuild reconstruction does not re-emit those anchors.
